Eligibility Criteria
The eligibility criteria to get the payout from this settlement require that you are a citizen of the US. Your personal information, such as name, SSN, date of birth, phone number, email address, etc., was compromised in the 2022 data breach of Mindpath Health.
Additionally, you received a notice about the data breach incident in January 2023 from the company and a notice about the settlement from the administrator, which carries your Notice ID and Confirmation Code; both are crucial for filing a claim form.
